Patient Registration

You may preregister with our office by filling out our secure and compliant online Patient Registration Form. On your first visit to our office, we will have your completed form available for your signature.

At the end of the Patient Registration form you will be able to upload pics, images, files, etc.

Very helpful and time-saving uploads include:

            Pics of both sides of your dental and medical cards

            Pics of both sides of your driver license

            A pic of your referral if you have it in paper form

Other helpful uploads would be: medication lists, medical records, or whatever else you think might be important.
